Your completed new patient form. If you have dental insurance, bring your insurance card too.

Do you see children?

Yes. We see patients of all ages, so the whole family can come to one office.

Do you take my insurance?

Is there parking?

Yes, there's plenty of parking at Fox Valley Center.

Is the office accessible?

Yes, the office is fully accessible.

Can I pay over time?

Yes. We offer payment plans through Cherry, with no hard credit check and 0% APR options available.

What if I have a dental emergency?

Call us. We see same-day emergencies. See dental emergencies.

I'm nervous about the dentist. Is that okay?

Completely. Many of our patients tell us they used to dread the dentist. Let us know when you call, and we'll take things at your pace and explain everything as we go.
